HOW TO APPLY FOR A MARRIAGE LICENSE OR CIVIL UNION

*** Do not sign the application before coming in, you must sign in front of us***
Where to apply:
1. If both applicants are residents of New Jersey you may submit the application where either applicant lives.
2. If one applicant is a resident of New Jersey but the other applicant is not you would appply in the town where the resident lives.
3. If neither applicant is from New Jersey, you apply in the town where the ceremony will be performed.
What to bring when you apply:
1. If either applicant has been divorced or had a marriage annulled, please bring the divorce decree or the civil annulment documents.
2. If your former spouse is deceased, please bring the death certificate.
3. A copy of your birth certificate, driver’s license, passport or state I.D.
4. Proof of residency.
5. Your social security card or social security number.*
6. A witness ( 18 years or older ) who knows both applicants.
7. The fee is $ 28.00. It may be paid by cash or check, if you pay by cash please bring exact change.
* Your social security number is required by law and will be kept confidential.
Any documents in foreign language must be accompanied by a certifed English translation.
Please call 30 days prior to the ceremony to make an appointment to apply for the license. Your appointment should be scheduled at least 7 to 10 days prior to the wedding date to comply with the mandatory 72 hour waiting period.

HOW TO OBTAIN A COPY OF A VITAL RECORD:
1. You must apply for the record in the municipality where the event occurred.
2. In order to receive a copy of the record you must be the subject, subject’s parents, legal guardian, legal representation, subject’s spouse, subject’s child, subject’s grandchild or subject’s sibling.
3. The copies are $ 10.00 per copy.
4. Please click here to download a copy of the Vital Record Request Application. You You may come in and fill out the form or have it mailed to you and mail it back and then pick up the record after 4 business days.
5. If you chose to mail in the application you must send in a copy of a photo ID with your current address on it. If you do not have a photo ID we will accept a copy of 2 of the following:
Non- photo driver’s license Green card
Vehicle Registration Passport
Insurance Card School ID
Voter Registration Utility Bills
